What We Offer

Our area of focus resides in chronic wound care. Collaborating with Skilled Nursing Facilities & Home Health Agencies to assist in the management & healing of chronic wound patients in the Tri-State Area by using advanced wound care technology. In most instances, we come in after all conventional measures have been exhausted. Our dedicated team comprised of a Certified Wound Care Nurse Practitioner and Wound Care Nurse can help your facility address an unmet need in chronic wound care management. Our team focuses on bringing the best quality of care to the patients in our region. 

Our Roles 

At Pinnacle Comprehensive Care, our team is dedicated to service the medical community to assess and treat chronic wounds in the Medicare Part B setting where the wound has aged greater than 28 days and less than 50% healed. We will collect all clinical information, patient consent as well as patients Medicare Part B insurance to verify benefits completing an IVR. Our practitioners are fully ensured and licensed taking the risk away from the Skilled Nursing Facilitiy and Home Health Agency.

Skilled Nursing Facilities

The Role of the Skilled Nursing Facility (SNF)

Your role is to simply contact us when you encounter a chronic wound that has documentation confirming the wound is greater than 28 days with less than 50% healing. From there, our medical team of Physicians, Nurse Practitioners & Wound Nurses will work directly with the patients Medicare Part B insurance plan to conduct a benefits investigation and work through any access and affordability issues with the goal of receiving an approved claim to treat the patient with one of our skin graph products.

Home Health Agency

The Role of the Home Health Agency

Your role is to simply contact us when you encounter a chronic wound documented 28 days with less than 50% healing. The initial documentation may have occurred while the patient was in the hospital prior to discharge. Our team will provide the agency a checklist to ensure all conventional wound healing measures have been attempted.

How We Partner

Our goal is to partner with Local Home Health Agency's once a wound becomes chronic. Our value proposition is to save the Home Health Agency Nursing Team some potential visits and more importantly healing the patients' chronic wound. This model provides the Home Health Agency one of our Practitioners who can help manage the wound using the latest skin graft technology. This protocol may save the Home Health Agency potential visits which reduces cost and more importantly expedites the healing of chronic wounds, providing a better patient experience.
